Crystal sickness (BPPV)

Crystal sickness or Benign paroxysmal positional vertigo (BPPV) is a common cause of dizziness and vertigo. Symptoms include sudden feelings of spinning, falling or "the world spinning" after certain head movements. The attacks can be severe, but are usually short. Individuals may experience difficulty standing, balancing, walking, and may also have nausea and vomiting. BPPV can vary in severity, and it can cause anxiety and frustration.

BPPV is relatively common and accounts for approximately 30% of diagnoses of dizziness. Up to 50% of people will experience BPPV during their lifetime, and the prevalence increases with age.

What are the effects of crystal sickness?

The effects of BPPV can vary. It can be a mild condition that goes away on its own, especially when the head is in certain positions. But in some cases, BPPV can persist and become a long-term problem, and it can lead to disability, especially when multiple channel systems are involved. The brain can adapt to the erroneous signals and ignore them, but some symptoms can persist, including anxiety, panic disorder, cognitive difficulties, headaches, memory problems and various other neurological symptoms.

BPPV occurs due to disturbances in the vestibular system of the inner ear. The otoconia crystals detach from the vestibule and flow into the semicircular canals where they do not belong. This disrupts the normal function of the canals and creates a false sense of movement and symptoms of BPPV.

Treatment

The treatment for BPPV involves repositioning maneuvers, moving the head through different positions to guide the crystals back to the right place. The Epley maneuver is a commonly used method that has revolutionized the treatment of dizziness and has brought relief to many people. Simple cases can be treated with a single Epley maneuver, while more complex cases may require multiple maneuvers. When other channels are involved, different types of repositioning maneuvers may be required.
